Create a temporary folder or subdirectory on your harddrive.

Click on the appropriate link to download the file. Your browser may ask you to "Save File..." or "Save it to disk" – choose that option and save the file to the temporary folder. Do NOT choose the "Open" or "Run" option!
Additional instructions for Q-Rate v4.20:
1.
Run the downloaded program (QR420.EXE) from a DOS prompt, Windows 95 Run option, or the Windows Explorer. This program will extract the Q-Rate files into the temporary folder.
2.
Run the install program (INSTALL.EXE) located in the temporary folder.
3.
After the install program runs successfully, you may delete the temporary folder and all the files contained in it.
4.
Run the program (QRATE.EXE) to launch Q-Rate.
Additional instructions for Direct Service Points (All Files):
1.
Unzip the downloaded file (ALL110C.ZIP or ALL110Z.ZIP). If you need the zip utilities, download them from PKWARE at www.pkware.com. This step will extract all of the ABFS 110 files. If you have downloaded both files, be sure to unzip each into a different folder or subdirectory.
2.
At this point you may access these files by importing them into a spreadsheet or database application, or you may view them with any word processor or text editor.
Additional instructions for Direct Service Points (by State):
1.
Unzip the downloaded file. If you need the zip utilities, download them from PKWARE at www.pkware.com. This step will extract the *.TXT file from the file downloaded.
2.
At this point you may access the *.TXT file by importing it into a spreadsheet or database application, or you may view it with any word processor or text editor.
